Gallbladder surgery in China: the middle path, real costs, and how to choose
There are two surgical routes for gallstones: cholecystectomy (removal) and gallbladder-preserving stone removal. Western doctors rarely offer the second. Here is the honest breakdown of both, what they cost at verified public prices, and how to choose — from a navigator who checks the paperwork, not a hospital sales team.
In the US, UK and EU, the default answer to symptomatic gallstones is one sentence: remove the gallbladder. The preservation option — open it up, take the stones out, keep the organ — exists in Western surgery but is rarely offered to patients, and often dismissed when mentioned.
In China, gallbladder-preserving stone removal (保胆取石) is widely available, mainly through private hospitals and the special-fee / international departments of public 3A hospitals. That combination — a procedure Western patients can't easily get, at prices a fraction of US out-of-pocket — is why people travel.
The honest caveat that most marketing leaves out: preservation has real eligibility gates and real recurrence risk. It is not "better" than removal for everyone. This page exists so you decide from data, not from hype on either side.
Laparoscopic removal of the whole gallbladder
- What it isThe gallbladder is removed entirely through small incisions. The standard treatment for symptomatic gallstones everywhere.
- EligibilityAlmost universal — including acute cholecystitis, large stones, thickened walls.
- Recovery1–2 nights in hospital; most people return to normal activity within 1–2 weeks.
- Watch-outsA minority of patients get post-cholecystectomy diarrhea (bile-salt related); it usually settles. No gallbladder afterwards, by definition.
Stones out, organ stays
- What it isThe surgeon opens the gallbladder (laparoscopically or via endoscopy), removes the stones, and closes it — the organ remains.
- Eligibility gatesNot for everyone. Real gates: good gallbladder function on a HIDA scan, no thick-walled gallbladder, not heavily sludge-filled, stone count and size within limits. Checked by ultrasound + HIDA before any surgeon should operate.
- RecoveryComparable to laparoscopic removal; the organ is kept, which is the point.
- Watch-outsRecurrence is real if your bile chemistry keeps making stones. Many Chinese programs pair surgery with post-op ursodeoxycholic acid (UDCA) and a follow-up schedule. Verify the recurrence protocol before you book.

- Locals with Shenzhen employee insurance: about 90% of in-hospital costs are covered at 3A hospitals under the city's rules Shenzhen government policy Q&A, 2026 — which is why a local's out-of-pocket total looks nothing like a foreign visitor's.
- Overseas patients: no local insurance → full self-pay at public rates, or higher at international / private departments. Materials (staplers, clips, mesh) can push the bill well past the base range — always ask for the materials line in writing.
- National direction: China's National Healthcare Security Administration (NHSA) is standardizing digestive-surgery price items nationwide (2025) — prices are converging toward published benchmarks, but city-by-city execution still differs National Healthcare Security Administration, Aug 2025.
Four filters, in order — the same ones we use on our Shenzhen and Guangzhou directories:
1. 3A rating first. China's official three-tier accreditation is the baseline filter. For a public option, stay inside 3A hospitals.
2. The specialty department. For gallbladder work you want the hepatobiliary surgery unit (肝胆外科) — ideally a National Clinical Key Specialty (国家临床重点专科). That designation means the national regulator has verified the department's volume and capability, not just its signboard.
3. International / special-fee department for foreigners. English-speaking staff, insurance direct-billing, structured English reports with reference ranges, and often published fee schedules. On the preservation route especially, these departments do most of the volume.
4. A written package quote before you pay. Ask in writing: does the price cover the hospital stay, anaesthesia, and every material? Which surgeon operates, and how many of this exact procedure has he or she done? What is the post-op protocol and follow-up schedule — and the recurrence plan?
- Ultrasound + HIDA before you fly. Preservation eligibility is decided by imaging, not by marketing. Bad ejection fraction, thick walls or heavy sludge → no reputable surgeon should preserve.
- Ask for the full package in writing — surgery, anaesthesia, hospital stay, materials, and what happens if complications extend the stay.
- Ask for the English report spec — units and reference ranges on every value (the #1 reason overseas reports get re-tested). See our post on GP-ready reports.
- Know the surgeon's name and volume for this exact procedure — not just "the department does many of these".
- Get the recurrence protocol in writing — post-op UDCA (or not), imaging follow-up schedule, and who to contact if symptoms return after you're home.
